Consent Forms
If you're booking an appointment for a minor, there are a few requirements to take care of beforehand. A consent form and notarized consent form are required, a parent or legal guardian must be present for the entire appointment, and both the minor and guardian must bring valid photo ID. Minors should also bring a birth certificate.
In-House Notary: We offer in-house notary services at our Beach Blvd location Monday through Saturday from 12 PM to 8 PM for a $10 fee. Consent forms are available on our website. If you have any questions, feel free to contact us.

Clients ages 16 to 17 must complete a Tattoo Consent Form and a Notarized Consent Form, with a parent or legal guardian present for the entire session and valid ID required from both parties.

Anyone under 18 must complete a Piercing Consent Form and a Notarized Consent Form, with a parent or legal guardian present for the entire appointment and valid ID required from both the minor and guardian.
